This form is to be used for anonymous reporting of suspected child maltreatment, injury, medical condition, and/or program violation or sexual harrassment.

The North Carolina Division of Social Services (DSS) defines child abuse as “the intentional maltreatment of a child and that can be physical, sexual, or emotional in nature.” DSS defines neglect as “the failure to give children the necessary care they need.”

Both as a matter of University policy and North Carolina State law, any member of the University Community who reasonably suspects that a minor has been abused or neglected by a parent, guardian, caregiver, or staff member has an absolute obligation to report that suspicion to the appropriate County Department of Social Services (“DSS”). The Orange County (North Carolina) DSS is the appropriate contact for incidents occurring on the University’s campus in Chapel Hill, regardless of the residence of the minor involved.

If you are a covered program operating under the Protection of Minors policy, you cannot make an anonymous report. You must report the incident to the Orange County DSS. If you are not sure whether you can make an anonymous report, please contact the Protection of Minors on Campus Office at 919-843-8995 or protectionofminors@unc.edu.

If the child is in immediate danger of additional harm, injury, etc., please call 911.
